I invite you to schedule an initial consultation. During this session, we will discuss your overall health, current nutritional intake, laboratory data pertinent to your diagnosis, medication-nutrient interactions, evaluate your nutritional status, and determine how I can best support you in achieving your goals by creating a personalized nutrition plan backed by evidenced-based science to improve your health.  Viva Nutrition offers both in-person or Telehealth consultations.

Initial Consultation

Each client starts with a medical nutrition consultation.  During this appointment, we will discuss your nutritional intake, medical history, exercise routine, food preferences and more.  This allows us to design a nutrition tailored to your specific health and nutrition needs and goal.

Initial Consult Fee: $150 (60 min)

Follow Up Appointments

Follow up appointments include individualized nutrition guidance to focus on progress, discussion on recent lab data, food tolerance, preferences and nutritional intake.

Follow Up Fee: $100

Insurance

Viva Nutrition accepts the following insurances:

  • Regence

  • Health Management Administrators

  • Blue Cross Blue Shield

  • UnitedHealthcare

  • Premera

  • Lifewise

  • First Choice Health

  • Medicare (referral required)  Medicare covers Medical Nutrition Therapy for Chronic Kidney Disease GFR 50-13, Kidney Transplant within the past 36 months and Diabetes.

  • WA State Medicaid (coverage depends on specific plan)

  • WA State Molina

Insurance is not a guarantee for nutrition counseling coverage.

Please call your insurance provider to verify coverage before appointment time.  Viva Nutrition does not call the insurance provider to guarantee coverage.  Recommended questions to ask insurance.

  • Does my insurance plan cover outpatient nutrition counseling? Medical nutrition therapy? Preventative nutrition counseling? Dietary counseling and surveillance?

  • Is there an out-of-pocket max or a deductible I must first meet?

  • Is a referral required?

  • Are there specific diagnosis code restrictions?

  • How many sessions (or units) am I eligible for?

Forms of payment accepted

Forms of payment accepted include: cash, check, credit cards, debit cards, and HSA/FSA accounts. You are responsible for payment in full before or at the start of your appointment. Please make checks payable to "Viva Nutrition Counseling". 

 

Cancellation Policy

If you cannot attend a scheduled session, please call to cancel or reschedule within 48 hours prior to appointment.  If you are late for an appointment, the appointment will still end on time out of respect for the client scheduled after you.
